目的 分析甲状腺功能紊乱(TD)孕妇实施血清甲状腺抗体过氧化物酶抗体(TPOAb)联合促甲状腺激素(TSH)检测的诊断价值。方法 回顾性分析84例孕妇的临床筛查结果。孕妇均接受TPOAb、TSH检测，分析其筛查结果；其中非TD孕妇42例作为对照组，TD孕妇42例作为观察组，均经病理检查确诊。对比两组孕妇TPOAb、TSH变化及诊断效能。结果 两组TPOAb、TSH水平相比，观察组TPOAb、TSH水平均高于对照组(P<0.05)。联合检测敏感度、特异度、准确度均高于单项检测(P<0.05)；联合检测漏诊率、误诊率低于单项检测(P<0.05)。结论 在妊娠期TD诊断中，采取TPOAb、TSH联合检测方法，可提高诊断效能。

Objective To analyze the diagnostic value of serum thyroid antibody peroxidase antibody(TPOAb)combined with thyroid stimulating hormone(TSH)testing in pregnant women with thyroid dysfunction(TD).Methods Retrospective analysis of clinical screening results of 84 pregnant women.Pregnant women undergo TPOAb and TSH testing to analyze their screening results;Among them,42 non TD pregnant women were used as the control group,and 42 TD pregnant women were used as the observation group,all of whom were diagnosed by pathological examination.Compare the changes and diagnostic efficacy of TPOAb and TSH in two groups of pregnant women.Results Compared with the control group,the observation group had higher levels of TPOAb and TSH(P<0.05).The sensitivity,specificity,and accuracy of combined detection were higher than those of single detection(P<0.05);The missed diagnosis rate and misdiagnosis rate of joint testing were lower than those of single testing(P<0.05).Conclusion In the diagnosis of TD in pregnancy,TPOAb and TSH combined detection can improve the diagnostic efficiency.
